NOTE: 2025-26 Medical Forms have not been released (as of 06/27/2025). See below for more information about the most recent paperwork update. For any questions, please contact the athletic training staff for any questions.
Welcome to the University of the South Athletic Training webpage. The sports medicine clinic is located on the bottom floor of the Fowler Sport and Fitness Center. The spacious 2,000 square foot athletic training room includes six treatment tables, six taping stations, a rehabilitation area, three whirlpools, a physician's office, and offices for the Sports Medicine staff. The co-ed athletic training room handles all the medical needs for over 550 student-athletes participating in 24 NCAA Division III varsity sports.
As a student-athlete, if you are sick or injured, we ask that you report to the sports medicine clinic and our staff will thoroughly evaluate your condition. Our staff will work directly with you to identify the cause of your injury and formulate a plan to ensure that when you return to competition, it will not only be safe for you to return, but you will be able to perform at a level that you were prior to your injury.
2025-26 Paperwork update from Dr.Serrano:
Our new platform is still in production. This is where all your documents (physical, sickle cell, ADD/ADHD paperwork, insurance card(s), and government-issued photo ID) will be stored. It is also where all the forms you need to sign will be. These forms will be digital, so you won't need to print them to sign them. The only items you would need to print are the physical, sickle cell test result form and the ADD/ADHD form (if applicable); all of those are attached to this email. Do not complete the packet on the athletics website, as these will not be accepted. Hopefully, I will have more information about when it will be available this coming week.
What does this mean for you? It means that paperwork for next year has not been released yet. Once it is available, you will receive an email with more information.
Is there anything you can do in the meantime? Yes there is.
- Get your Physical scheduled: (must be signed by an MD or DO; you will also need to send this to the University); the form is attached. The form says that there are other pages needed; you only need the form attached above. The information from the other pages will be entered in the new platform.
- Get your Sickle Cell Test: (this is a requirement for ALL student-athletes; there is no waiver for this; if you were tested at birth, you can submit these results as long as your name and date of birth are on it); you can probably get this done at the same time as your physical; the form is attached to this email.
- Take medication for ADD/ADHD? Please make an appointment with your prescribing physician to get the attached form completed. It must be signed by the prescribing physician and include the required documentation; this is only for those who take medication for ADD/ADHD, and it is an annual requirement for those individuals. The form is attached.
- FOOTBALL: Please make sure to get your annual physical (form is attached to this email). This is an annual requirement for all members of the football team.
- Non-Football Athletes: While a physical is not required this year, you are welcome and encouraged to get one as part of your pre-participation paperwork. This is extremely likely to become a mandatory requirement next year.
- Make sure you have a clear picture of the front and back of your insurance card ready to go when paperwork is released. If your insurance will be changing, that is completely fine, and you can update it when it renews/changes.
- Make sure you have a clear photo of the front and back of a VALID government-issued photo ID ready. This includes your driver's license or passport.
These forms must be dated after June 1st. Please hold on to these documents until I email you all with the next steps. I will send out more information when the paperwork is available for you all to complete.
Deadline: For first-year student-athletes, the deadline for paperwork is July 15th for ALL incoming/new students to submit their paperwork to the University. Please make sure to upload it to your health portal. For everyone: with July 15th looming, the deadline for paperwork will be extended to August 1, 2025. Hopefully, this will be enough time for you all to get set up in the system and get your paperwork done. If this needs to be adjusted again, I will let you all know.
UPDATE: Athletic Training is now on an appointment-based schedule.
Sports that are in season can come in for treatment before practice/games, but if they need any treatment or rehab throughout the day, they have to make an appointment.
